Medication Errors in Pharmacy: An Update
Dispensing errors. Medication errors. Adverse drug events. Drug misadventures. Dreaded words that no conscientious pharmacist wants to hear. Each of these terms is a pharmacist's boogeyman, hiding beneath the counting trays, flitting amongst the drug bottles on the shelves, hovering above every distraction in a pharmacy. It is enough to send a chill up a pharmacist's spine. Unfortunately, mistakes and errors do happen. It is the nature of the beast, a health care system that has flaws creating the opportunity for error. This course describes the efforts underway to correct the system, and to help focus less blame on the individual and more on the system.
